Will ettl and off camera flash for insects work in dark dense vegetation, combined with the 400 end of the 100/400 as opposed to a dedicated macro and flash?

Yes, flash lighting is just a way of providing light, the focal length of the lens doesn't matter. BUT the illumination is determined by the flash to subject distance. Using a telephoto means you need to use off-camera flash both to make sure the lighting is on the subject and to avoid lens-shadow because with flash on/near to the camera the lens will get in the way. Lighting anything hidden in dense vegetation will always be a challenge.
