When the Passport Office wanted monochrome I used to use a 6x6 TLR, make a contact print and trim to size. Last time I did my own passport photo with a Canon G5, on a tripod, screen rotated so I could frame & adjust pose from the front, exposed using the wireless remote control that came with it. Scaled to the right size in Photoshop Elements and printed on Epson Premium Glossy. Passport Office were happy enough.
